How you can prepare for your appointment!

  • Be aware of any pre-appointment restrictions. When making your appointment, make sure to ask if there is anything you need to do prior to your appointment, such as restrict your diet.
  • Be sure to bring your insurance card and ID, and payment for your co-pay, if applicable.
  • Unless your primary care doctor’s office already sent us results from all blood and urine tests and copies of any imaging studies, we request you bring them so that the Urologist will have access to pertinent tests and results.
  • List any symptoms you’re experiencing
  • Are you experiencing any major stresses or recent life changes?
  • Bring a list of all medications, vitamins or supplements that you’re taking.
  • List any questions to ask your Urologist.
  • If relevant, bring a diary of your daily beverage and food intake.
